Le programme comprend deux modalit\u00e9s, y compris la surveillance \u00e0 distance des patients et l\u2019appel automatis\u00e9. \u00c0 l\u2019int\u00e9rieur du programme de soins cardiaques virtuels, il y a des services pour les patients apr\u00e8s-op\u00e9ration, les patients atteints d\u2019arythmie, le renoncement au tabac, le syndrome coronarien aigu et les patients souffrant d\u2019insuffisance cardiaque. Ce programme permet d\u2019attribuer la technologie appropri\u00e9e au bon patient. <\/p>\n<p><strong>Programme de soins virtuels pour l\u2019insuffisance cardiaque\u202f: Surveillance \u00e0 domicile (3 \u00e0 4\u202fpremiers mois) <\/strong><br \/>\nLe programme de surveillance \u00e0 domicile pour l\u2019insuffisance cardiaque est l\u2019un des programmes sp\u00e9cialement con\u00e7us pour les patients qui ont besoin d\u2019une surveillance quotidienne. Ce mod\u00e8le de soins actifs est bas\u00e9 sur un mod\u00e8le de soins de transition et fournit des soins apr\u00e8s le cong\u00e9 et entre les visites m\u00e9dicales lorsque les patients sont \u00e0 risque. Tous les fournisseurs de soins de sant\u00e9 de l\u2019ICUO peuvent orienter les patients vers ce programme. Une ordonnance ou une recommandation du m\u00e9decin est requise pour les h\u00f4pitaux communautaires. Ce mod\u00e8le n\u2019exige aucune visite \u00e0 domicile ni aucune indemnit\u00e9 de l\u2019Assurance-sant\u00e9 de l\u2019Ontario. L\u2019inscription prend de 5 \u00e0 10\u202fminutes \u00e0 l\u2019h\u00f4pital, o\u00f9 les patients apprennent \u00e0 installer et \u00e0 utiliser l\u2019\u00e9quipement avant de rentrer chez eux. Les patients signent une entente avec une infirmi\u00e8re afin qu\u2019ils transmettent leurs signes vitaux quotidiennement \u00e0 un moment donn\u00e9.  <\/p>\n<p>Le programme de surveillance \u00e0 domicile utilise des appareils qui poss\u00e8dent des enregistrements vocaux et fournit des instructions simples pour s\u2019assurer que les signes vitaux et d\u2019autres renseignements sont bien saisis. Les renseignements sur les signes vitaux sont ensuite transmis automatiquement \u00e0 un poste central de l\u2019ICUO, o\u00f9 ils sont examin\u00e9s par des infirmi\u00e8res sp\u00e9cialis\u00e9es en cardiologie. Program (CVC) is designed to provide virtual care to cardiac patients. The program is comprised of two modalities, including remote patient monitoring and automated calling.\r\n\r\nThis program is only eligible for the following patients:\r\n<ul>\r\n \t<li>Heart failure (HF) patients with one readmission\/one month or two readmissions\/in six months (NYHA III\/IV)<\/li>\r\n \t<li>Patients with new HF diagnosis<\/li>\r\n \t<li>Patients recovering from cardiac surgery<\/li>\r\n \t<li>Patients requiring vital sign\/ arrhythmia monitoring<\/li>\r\n \t<li>Any cardiac patient requiring frequent monitoring or trending of information to facilitate optimal clinical management<\/li>\r\n<\/ul>\r\nA fundamental part of this program is to ensure that the right technology is assigned to the right patient.\r\n\r\n<strong>Heart Failure Virtual Care Program: Telehome Monitoring (first 3 to 4 months) <\/strong>\r\nThe Telehome Monitoring Program for Heart Failure, is one of the specifically designed programs for patients who require daily monitoring. This acute-intervention model is based on a transitional care model and provides care after discharge and between medical visits when patients are at risk. All UOHI health care providers can refer patients to this program. An MD order\/referral is required for community hospitals. This model does not require any home visits, or any OHIP fees. Enrollment takes 5 to 10 minutes in the hospital, where patients are taught to install and use the equipment before going home. Patients sign an agreement with a nurse that they will transmit their vitals daily at a specific time.\r\n\r\nThe Telehome Monitoring Program uses devices which have recorded voice prompts and provides simple instructions to ensure vital signs and other information are captured properly. Vital-sign information is then transmitted automatically to a central station at UOHI, where it is reviewed by expert cardiac nurses. Nurses call patients for further assessment and intervention, as required. Patients are also asked several clinical questions; responses are priority coded as red, yellow and green priority and they receive interventions based on need.\r\n\r\nFor patients identified in community hospitals, a local contact nurse provides them with the equipment and training. An UOHI expert nurse follows the patient and communicates with their physician through information shared on the referral form. UOHI also trains hospital staff and provides ongoing educational support and equipment support, as needed.\r\n\r\n<strong>Automated Calling \/ Interactive Voice Response Model (IVR) (1 to 12 months) <\/strong>\r\nThis model uses a TelAsk system that automatically calls or texts patients every two weeks to ensure ongoing educational support. Questions are posed in a \u201cyes\u201d or \u201cno\u201d format and captured either through voice-to-text or key pads on the phone. Depending on responses, a clinician will be flagged to follow up with a call, as needed.\r\n\r\nPatients who have had cardiac surgery at UOHI are automatically enrolled into the program. The IVR model runs during regular work hours. However, patients can contact a nursing coordinator at any time during the call. If a patient needs immediate assistance, UOHI has a nursing coordinator available 24\/7 (not specific to the model).\r\n\r\n<strong>Technology Type:<\/strong>\r\nPatient Remote Monitoring\r\nUses Resideo\u2019s Genesis Touch on a Samsung Galaxy Tablet monitor with additional devices as necessary including Bluetooth enabled; blood pressure cuff, weigh scale, pulse oximeter, and ECG Cards.\r\n\r\nClinician Remote Monitoring\r\nClinicians use a dashboard to view patient data gathered from the aforementioned devices. Through this dashboard they can: receive alerts and review responses to questions, see trending reports and clinicians can select questions for patients to answer from a base of 150 questions.\r\n\r\nAutomated Calling \/ IVR\r\nWith the Automated Calling system, the patient receives a call, email or text. Model<\/h2>\r\nThe UOHI Cardiac model requires dedicated staffing to support the operational model:  \r\n<ul>\r\n\t<li>1 Advance Practice Nurse <\/li>\r\n\t<li>2 Registered Nurses <\/li>\r\n\t<li>1 Part Time Registered Nurse <\/li>\r\n\t<li>1 F\/T clerk <\/li>\r\n\t<li>1 Medical Lead  <\/li>\r\n<\/ul>\r\n\r\n2.5 full-time equivalents (FTEs) were trained to monitor patients either via remote patient monitoring program or via IVR responses and to interact with them virtually.\r\n\r\nOne registered nurse can monitor and manage 75 patients a day. \r\n\r\nThe UOHI Cardiac Telehealth Program was initially funded through an innovation Grant (The Richard Ivey Foundation).  Additional funding was received through the Aging at Home initiative, which enabled expansion to other hospitals within the LHIN.  \r\n\r\nWhen using automated calling you need to minimize the number of algorithm questions to keep patients engaged.\r\n\r\nIntegrating remote-monitoring data into an EMR is key to ensuring that physicians are on board and have access to real-time patient patients have been followed to date. \r\nIn October 2019:  \r\n<ul>\r\n\t<li>104 patients are on home monitoring<\/li>\r\n\t<li>1,924 patients are being followed by IVR<\/li>\r\n <\/ul>\r\n\r\nUOHI\u2019s Telehealth Monitoring Model: \r\n<ul>\r\n\t<li>300 sites across North America have adopted a model similar to UOHI\u2019s automated calling systems<\/li>\r\n\t<li>Similar
